Information we need from each person:
As a first step, our doctors will need medical information from each person who wishes to receive the vaccine, in order to assess whether it is appropriate and relevant in each case.
- A list of previous vaccinations (as detailed as possible, if exact information is not available)
- Information about current health status
- Any known allergies
- Any previous reactions to vaccinations
Personal details required to issue a prescription:
- Full name
- Nationality
- Passport / NIE / DNI number
- Email adress
Each person will also need to complete our GDPR consent form. If we do not already have a valid form on file, this will be emailed to you to fill in.
Next step
1. Medical review and prescription
Once we have received your information, the doctor will review your case. If appropriate, a prescription for the vaccine will be issued.
2. Booking your appointmen
Collecting your vaccine
Please collect the vaccine from the pharmacy prior to your appointment.
You may either bring it with you to your appointment or drop it off at the clinic in advance. The vaccine must be kept refrigerated (2–8°C) at all times.
